"if your insurance doesn’t have comparable services in-network you can try requesting they cover out-of-network providers at the in-network rate. The worst they can do is say no. I just learned this recently, so thought I’d share."

hmm interesting. we are lucky in that we have only a $100 deductible which we have already me. My son gets OT at school and I wish I could say it helped. He wore a weighted vest yesterday and still was completely distracted, unable to focus, or sit still. His teacher basically begged me to try medications. I am so torn. I know that in the case of basic brain chemistry, like in depression, the medications are a necessity.

It may just be the asynchronous development that is common for gifted children. I know there are many of C's behaviors I feel I can attribute to that along with overexcitabilities. Even if these features are common in gifted children, I can't help but wonder if they don't still warrant support of some kind. Not medication, of course, but services like PT and OT when needed plus accommodations in school, etc. I wish there were more research out there about this. For example, should overexcitabilities be treated like SPD? Are they really different? Or are we just giving the same set of behaviors two different names? Maybe overexcitabilities are just SPD in gifted children!

I can sympathize about the hefty co-pays. For us to go out of network, we'd have to pay over $1500 for a deductible and then have a 45% co-pay. I was also recently told that while our insurance would cover the other 55%, they only cover 55% of their customary charges. So if the place charges more then we need to cover the difference. At least this is what the latest rep told me. It's crazy. I'm still waiting to hear whether they've been able to work out an in-network rate with CHOP or the neuropsychologist. I can say, though, that CHOP said they would try to work something out with insurance. If your insurance doesn't have comparable services in-network you can try requesting they cover out-of-network providers at the in-network rate. The worst they can do is say no.
we are out of network which will cost us 30%. At the last neuro as I recall that was about $400 and I believe at CHOP the initial eval is $1400, so we are looking at a big chunk again.

Yes my son has similar behavior at home. LIke you, we quasi home school and as he concentrates he will sometimes actually wiggle MORE as he struggles to figure something out. Unlike your son he has a fast processing time, so he gets bored if teaching doesn't go fast enough. He loves self paced learning for that reason. We use a lot of online learning sites for that reason.

I honestly don't know where I fall on the ADHD diagnosis. If he has it, then it is on the impulsive/hyperactive side, but I do see lots of development in his executive functions this year, so part of me thinks it may be a maturity issue.

Please stop back and let me know about your experience at CHOP. I'm waiting to hear back from my insurance whether they will cover CHOP or a local neuropsychologist I found, since they were unable to provide any viable in-network options. I hope CHOP is helpful in figuring out whether your son actually does have ADHD. If they are any good, they will take your son's giftedness and educational environment into consideration. Unfortunately, I only know their ADHD clinic by reputation and not from experience.

Does your son have ADHD behaviors at home or just at school? Is he able to focus better when he is challenged? I was initially very reluctant to even consider ADHD for my son. However, his inattentive behaviors have increased dramatically this year both at school and home. He now seems to shy away from challenging work as well. Whatever his diagnosis may be, I know we need to do something.

I recently finished reading Parenting Children with ADHD. I plan on copying a page and taking it with me to the pediatrician that mentions other issues that can cause inattention. Some simple blood tests can rule many of them out. Hopefully some homework on my part can minimize the chance of a misdiagnosis.

just finished reading through your blog and there are so many things similar about our son's right down to the age, but also giftedness, SPD, OE etc. We live on the other side of the bridge however, which is much different, since there are no gifted IEPs. We have had one under other health impairment since Jan of pre K. I'm heading to CHOP because I'm so tired of hearing from the school that my son must have ADHD while the two neurologists I've taken him two have said that his behavior is not that uncommon for high IQ kids who are bored in school.

You make some very good points about starting the OT and PT now. I probably should have given it more consideration. If they were able to offer him services at his school, I don't think we would have hesitated to start now. The logistics of having him get the services at a different school, plus knowing that his teacher would not be in the loop, made us wonder if it was worthwhile for such a short amount of time. I asked whether they would be willing to start the services in the summer, but didn't receive a reply. Meanwhile, I got the paperwork today stating a start date of 9/7, so I guess that means "no".

C's grades are very good, but it's mainly due to the work being so easy for him. It doesn't require much sustained attention or effort. If he were given challenging work then I think his grades would tank. Supposedly our district will challenge him next year with a GIEP in place. I'm not sure I really believe it, but wonder how C will manage in that kind of environment if so.

Were there any specific things you did for your boys while you were waiting for for them to see the neuro ped? Were they already receiving services? It's hard to imagine waiting a year to get any answers, but it looks like we may not have a choice. Your post at least makes me hopeful that the 504 will help!

What a shame you are having so much trouble.. I would not wait until the next school year to start his therapies.. Its never "too late." Even three or 4 weeks of therpies/ intergrations will be helpful, and he will then know the routine when he goes into the next grade. I have two boys, gifted, aspergers w/spd. And the oldest has behavior issues at school, so here came the 504! It has been helpful, as gifted kids fall through the cracks. Special Ed is not what it was in the 70's. My youngest has this,(OT & Speech) due to inability to follow directions, transitions, etc. he is making great progess. If your sons' grades are excellent, and you are talking a behavior issue, don't hold him back...The 504 will be able to address the behavior issues, and he will get the one on one he needs at TIMES. As far as a label, you NEED THE DIAGNOSIS, not to label your son, but to help him. It took a year to get both my boys into see a Neuro Ped., but I am glad we did. Has made a huge difference in their "quality of life" issues, and the help they now receive.
